Pilot program with Community Rehabilitation Service Support Centre (CRSSC) to enhance quality of care for SCH disabled residents

Introduction :
Community Rehabilitation Service Support Centre (CRSSC) in QEH provided advance technology service to suitable patients. Disabled Unit of Cheshire Home, Shatin (SCH) is the first Infirmary hospital in Hong Kong to run the 6 months’ pilot program with CRSSC to enhance quality of life of SCH residents via interdisciplinary applications of innovative technology.
Objectives :
1)Developed collaboration between SCH with CRSSC via the half year pilot program
2)SCH multidisciplinary team cooperated with CRSSC team to provide innovative assistive devices to SCH residents
3)To enhance the residents’ mobility independence and self-care ability
4)Evaluated and identified area for improvement in the pilot program
Methodology :
1)SCH ward manager coordinated with multidisciplinary team members including doctors, nurses, physiotherapists, and occupational therapists, recruited suitable residents to join the CRSSC pilot program.
2)CRSSC therapist came to SCH once per week to provide assessment and identified suitable devices for patients, which included 3 feeding devices, 1 door opener, 1 standing wheelchair and 2 communication devices.
3)Provided training of assistive device to patient and staff
4)Reassessed the use of device by SCH multidisciplinary staff and provided feedbacks to CRSSC
5)CRSSC provided adjustment accordingly
6)Assistive devices were provided to the patients
Result & Outcome :
Total 4 cases (3 males and 1 female, age from 38-64 years old) were benefit in the pilot program with 7 devices. A staff survey on the pilot program was conducted in Dec 2020. Total 32 questionnaires out of 40 questionnaires were received including 18 nurses, 14 patient care assistances. 87.5% agreed the assistive devices to patient were innovative and suitable. 90% agreed the program enhanced residents’ mobility, self-care ability and mobility independence. 96% stated the program improves resident’s quality of life. 81% of agreed the programs helps to increase job satisfactory. 78% of them expressed the assistive devices were easy to be used and setting up. 87% agreed the program is effective. 90% of them expressed the program is worth to continue after the pilot. The overall satisfaction from scored 0 to 5, the average score is 3.9.
The program is a milestone for collaboration with CRSSC in providing innovative devices for SCH residents. The self-care ability, mobility and dignity of patients are enhanced. It is worth to continue the program after the pilot period.
