Introduction :
All staff have to ensure all transactions of dangerous drugs in Wards are made according to relevant legislative requirements and operational guidelines. There is a mechanism to document and trace back each transaction of dangerous drug in HKWC.
Objectives :
To establish standardized mechanism to safe handling of dangerous drugs smartly
Methodology :
HKWC Medication Safety Committee has standardized the practice of handling of dangerous drugs in ward and clinic settings in HKWC since 2016 and on-going improvement measures is implemented Daily operation 1. Established Guidelines on Handling of Dangerous Drugs in Hong Kong West Cluster (HKWC) - Version 2 2. Documentation for different types of transactions were standardized and samples of all situation of transaction was established for staff reference, such as how to document 3. Explicit instruction to show that who and how to dispose the unserviceable DD, DD can be disposed DD by nurse after administration, otherwise, all DD should be disposed with pharmacist
Smart and safe storage of DD
4. Shift in-charge should be a responsible staff to keep the key of DD cupboard to ensure security of DD and should be involved in DD checking 5. A key chain of Key was provided to each ward to facilitate handling of DD key 6. In order to escalate the security of DD storage in wards in which frequency of DD transaction was high, Smart Drug Cabinets were equipped in two acute medical call wards to store DD Securely Check and balance 7. Twice weekly to inspect DD balance and monthly check and trace back DD records and stock 8. Fixed a date in each year to update a list of authorized nursing staff with specimen signatures to pharmacy. 9. align with HAHO direction, yearly self – assessment of handling of DD is done to arose staff awareness of safe handling of DD Incident management 10. Established HKWC Guidance Notes on Incident Reporting of Dangerous Drugs Irregularities
Result & Outcome :
A comprehensive smart and safe Dangerous drug management is established in HKWC, the incident of DD irregularities from 4 in 2018 to 1 in 2020 in HKWC. Way forward To explore feasibility of equipping more smart drug cabinet in high risk areas, such as AICU, OTS.
