Introduction :
CMS has been used for more than 30 years. It is a genius system to capture the health condition of patient. There is a big data collected by CMS. Practically for daily ward activity, we need to have a system to reflect the patient journey that the Department/Ward management team to get more concrete and cemented information of the patient/ward situation. The data collected can enhance the manpower allocation in dealing with the daily workload.



EAS is a system to manage the patient journey from admission to discharge. It will help to collect basic data of patient and display in the dashboard for staff reference. The dashboard displays the information about the Case Resident, subspecialty bed capacity, defined long stay case number, Reh ward booking, etc. Even as on request to display the stroke days. All of this functions provide reference for Ward Physician/IC to view the hurdle before discharge.

The patient number of Resident and the SMS admission call provide information for management team to consider the workload of frontline.
Objectives :
In-crop with organizational computer system to simply the clerical work

Re-organize the background data for management review

Instant tracking of patient discharge support

Minimize the clerical work of nurse/clerk in ward
Methodology :
The system implemented in 3B. Team member work closely with the vendor to sketch out the basic system and dashboard display. Confirm the basic structure of doctor’s duty and convert it digitalized. Vendor set up a local network for basic system and run parallels with the current practice Collect innovative/improvement idea from doctor, nurse and clerk about the system periodically. Prompt update the system via communication between stakeholder. Promulgate in Department and collect feedback of system design.
Result & Outcome :
System runs with local network and will be run in HA wifi in Dec. Patient admits and assigns to designate Resident automatically and displays on dashboard with the below features.

 Days of Stroke,

 the number of case under Resident,

 LOS >14 days,

 the readiness of Patient for NEATS.

 The on call list of Doctor to the designated ward.

The accuracy of the queueing system has been tested with satisfactory result. Dashboard displays the information of patient journey. We will step forward to run the program in another General Acute Ward in phase 2 and at times to explore the IC Handover Module to enhance the patient journey display.
