Screening for Fitness to Drive Assessment from Acute Stroke Unit (ASU) in Tseung Kwan O Hospital – A Pilot Enhancement

Introduction :
Driving, a complex instrument activity of daily living (IADL), is an essential occupation for drivers. Previously, it depended on patients’ self-reporting of their role as drivers in ASU to initiate subsequent referrals for formal driving assessment by Occupational therapists (OTs). There are various limitations causing under-utilization of the service. Therefore, enhancement work was implemented to improve the quality of the driving assessment service in 2020.
Objectives :
To raise the awareness and knowledge of OTs in driving assessment for stroke rehabilitation. To better identify TKOH ASU patients with the need of driving assessment. To improve communication and documentation for patients’ driving need and doctors’ decision of referring driving assessment.
Methodology :
In mid-2020, a tailor-made educational sharing was conducted to OTs in TKOH. Moreover, a proactive approach was adopted in identifying patients with valid driving licenses during initial intake of patients in ASU. A new form was designed to communicate with case doctor for endorsement, document patients’ driving issue and plan.
Result & Outcome :
Post-sharing evaluation from 5 OTs treating ASU patients in TKOH showed that they perceived better understanding of their role in driving rehabilitation with self-rated score increased from 4 to 8.25 out of 10. After adopting the proactive approach, 177 new patients in ASU were screened since 7.2020 while 17 patients were eventually referred for formal driving assessment. There was a 54.5 % increase in patients referred compared to last year. The newly designed form was used in daily documentation in ASU for patients with valid driving licenses. The form received positive feedback from nursing and medical colleagues for better communication in clinical practice.
