Introduction :
Ward 2C is a geriatric and stroke unit, almost most of patients are elderly with poor mobility, such as bedridden, four limbs contracture and so on. Besides, limbs paralysis and even hemiplegia are main clinical presentation in severe stroke patient, pressure injury could be resulted. Therefore, wound and skin assessment is one of the issues that we must pay attention during assessment. Manual recording, in particular, wound chart is commonly used in general ward nowadays, nurse usually record wounds and skin condition by illustrating the wound’s nature, type, integrity and size with handwriting. It is time consuming and inaccurate while patient has multiple wounds or unfamiliar skin condition.

In the light of that, the workshop of clinical photo promotion is aimed to maximize the record accuracy and multidisciplinary communication, concurrently, minimize paperwork and time use for nursing staff.
Objectives :
1. To enhance the usage of clinical photo in ward

2. Minimize time-consuming paperwork

3. To prevent such inaccurate assessment

4. Foster the effective communication to multidisciplinary teammate
Methodology :
The 30mins presentation was divided into four parts:



1. The introduction of clinical photo:

A brief introduction, including the use, precautions, purposes, was presented to all nursing staff by using PowerPoint



2. The demonstration of using clinical photo

Two nursing staff were paired up, a clinical iPad was allocated to each pair. Presenter demonstrated and explained the manipulation of clinical photo.



3. Re-demonstration

Participants re-demonstrated of photo-taking and editing in clinical camera and patient album.



4. Questionnaire for the opinion before using the function of clinical photo

Questionnaire was distributed to participants for collecting their opinions of clinical photo presentation.
Result & Outcome :
Two months after the presentation, the use of clinical photo function was boosted up significantly, and widely utilized as a communication tool with doctor and allied health colleague such as wound nurse and community nursing staff. Moreover, nursing staff now become more familiar to manipulate the clinical photo function in iPad, compared to previous. Most updated wound and skin condition can be observed via clinical iPad and CMS system, it is time-saving and more accurate to update and record the change of wound condition regularly, rather than using traditional paper record. Questionnaire was distributed to participants for collecting their opinions of clinical photo presentation.





Another questionnaire was distributed to nursing staff after 2 months of clinical photo presentation, in order to collect and organize the statistics of clinical photo usage in ward and opinions.
