Introduction :
Duplicated medications upon discharge from MMRC while patients have sufficient stock on-hand.
Patients are at risk of taking duplicated medications.
It is wasteful if duplicated drugs are dispensed.
Objectives :
To minimize duplicated medications upon discharge from MMRC.
To improve drug-adherence by providing bedside counseling.
To prevent harm from discharge discrepancies and promote patient safety.
Methodology :
Pharmacists screen the discharge prescriptions against patients’ medication record on EPR (Electronic Patient Record). If the patients have sufficient on-hand stock, pharmacy staff will contact nursing staff to clarify with the patient, or their relatives/carers. Those duplicated medications would not be dispensed.
Pharmacists review discharge medications and previous medications orders.
Bedside counseling will be provided to ensure medication compliance and enhance patients satisfaction by providing them better understanding about their medications.
Result & Outcome :
10% of the prescribed medications were saved from May to November 2020.
During bedside counseling,pharmacist demonstrated inhaler technique, explained the use and side effects of different kinds of laxatives and painkillers. Patients were appreciated with the explaining.
