Introduction :
According to WHO 2018, Falls are the second leading cause of accident or unintentional injury deaths worldwide. Each year an estimated 646000 individuals die from falls globally, and 37.3 million falls that are severe enough to require medical attention occur each year. Fall prevention strategies should be comprehensive and multifaceted. In N2 Medical ward, the fall rate is in increasing trend 0.90 / per 1000 bed days in Nov 2018 up to 1.69/ per 1000 bed days in May 2019. In light of this, The Fall Program has been reviewed and "RESTART"
Objectives :
Reduction of Fall Rate in N2 medical ward.
Methodology :
The Fall Program has been review and "RESTART". 1.Risk Assessment by MORSE FALL Scale on admission, consecutive 2 days after admission, weekly and whenever changing condition. 2.Equipment of using Fall Alarm Pad (1 pad 1 bed) since July 2019. 3.Study of the effectiveness of the Fall Alarm Pad. 4.Team effort on multidisplinary approach, such as woking with physiotherapist and occupational therapist to provide training to healthcare workers, relatives and patients, and conduct Environmental Inspection Round in clinical wards. 5.Awareness on fall prevention is promoted and Fall Prevention Awareness Week was held from 9-15 Sept 2019. 6.Root Cause Analysis on each fall incident, and rule out the possible contributing factors and solutions. 7.Training & Education to healthcare workers, patients and relatives on fall prevention including mobility assessment, Proactive Patrol Round, and exercise and physical activities .
Result & Outcome :
Fall Rate in N2 medical ward is in decreasing trend. The Fall Rate of N2 Medical ward is in decreasing trend from 1.69/per 1000 bed days in May 2019 down to 0.87/per 1000 bed days in October 2019, and Zero fall incident from November 2019 to April 2020.
