Introduction :
Patient safety and care quality are the primary objectives for hospitals, patients and their families. Concerns had been raised regarding patient’s fall incident in O&T with severity index: 4 in 2019 in the department. This quality improvement project was conceived to ensure the quality indicators e.g. prevention of fall and pressure ulcer were considered and documented during the ward round. Regular ward round is believed to outcome a safe and high-quality patient care in ward.
Objectives :
1. To improve patient safety by observe the safety measures during ward round 2. To enhance communication between patient and nurse 3. To empower junior staff in patient care management
Methodology :
The ward round was scheduled at a regular time in pm shift. The nurse-in-charge will accompany the case nurse, ward manager or DOM during ward round. The team would introduce themselves to the patient and ensure that they discuss their care with them. Patient care plan was used as checklist to ensure key components of the ward round are not overlooked. Nursing care plan would be reviewed to ensure clear and written care plans was well documented in the round.
Result & Outcome :
1. Staff will perform nursing round in each shift 2. Improvement of quality indicator 3. Improvement of patient satisfactory Staff Survey on implementation of Patient Safety Round Patient Satisfaction Survey
