Introduction :
Skin tear (ST) wound is common among the elderly in the community. The wound is characterized by partial thickness or full thickness of skin loss, resulting in bleeding and pain. According to the guideline of International Skin Tear Advisory Panel (ISTAP), standardization of using dressing material on different type of skin tear wounds will be beneficial on wound healing and may prevent secondary injury of the wound. Therefore, a program was introduced to the staff in Kowloon Hospital(KH) Community Nursing Service (CNS) department from March to April 2020.
Objectives :
To empower the knowledge of skin tear wound management among staff in KH CNS department. Also, to standardize the nursing practice of wound management based on the suggestion of ISTAP.
Methodology :
In this program, a workshop was established to the KH CNS staff. In this workshop, the classification of skin tear wound and the standardized international standard of skin tear wound management were introduced. Different types of skin tear wound were treated with different kinds of dressing material. The Unique “Arrow” signage on the dressing material prevents secondary injury on ST wounds. The management guideline was demonstrated by using power point slide show and a tailor-made poster. After this sharing, the staff will perform ST wound dressing with a pair of dummy hands based on the suggestion of ISTAP. Finally, an evaluation form was distributed to the staff for the comment of the workshop.
Result & Outcome :
Total 64 staff had participated in this workshop. An evaluation form was distributed to the staff after the workshop. All the staff were satisfied with the workshop. Another check list was used to evaluate the performance of the staff on ST wound management during the return demonstration section. After the workshop, all the staff were able to verbalize and identify the classification of the ST wound types. At the same time, they know how to choose the proper dressing materials on different type of ST wounds. In the return demonstration section, all the staff were able to mark the arrow signage correctly on the dressing material in order to prevent the secondary injury of ST wound during the removal of dressing material. To concluded, this program had successfully empowered the KH CNS staff on ST wound management. In the future, this program can be run in the community such as the Residential Care Homes for the Elderly (RCHE). Through this training program, the health care workers can provide better nursing care to the elderly and raise their quality of life.
