Staff empowerment and enhancement by resuscitation skill update with integration of simulation training, debriefing and hands-on exercise in FYKH

Introduction :
FYKH is an extended care hospital, with patient mainly transferred from QMH. Some of them might deteriorate, and even require resuscitation, after their transferal. There are one to two patients required resuscitation monthly in FYKH M&G. Every nursing staff of FYKH have AHA resuscitation qualification, but in view of ongoing advancement in resuscitation skill, it might be difficult for every one of them to have most updated resuscitation skill. Hence a committed team with qualified simulation educator and AHA BLS instructor was built in 2019, strived to provide updated resuscitation knowledge through integration of traditional teaching, tailor made simulation video demonstration, group debriefing and hands-on resuscitation exercise. The aim is to enhance professional skills and teamwork of FYKH nursing staff and ultimately enhance patient centered care.
Objectives :
To enhance and empower our nursing staff to perform high quality resuscitation.
Methodology :
The course was divided into two parts: 1. Theoretical classroom based learning: a. A movie demonstrating common errors during CPR followed by an interactive discussion by a simulation educator. b. A knowledge and skill based teaching on performing high quality CPR by a BLS instructor. 2. Simulation training with scenario and hands-on exercise: a. Give a scenario to trainees, facilitate their learning process by putting knowledge and skills together into a real hands-on practice under a true-to-life learning environment. b. Debriefing after scenario will encourage trainees to have self-reflection and discussion. Hence, trainees will consolidate their learning experience and outcome. c. Each simulation has around 6-8 nursing staff In order to ensure most nursing staff having chance to attend the course. We applied learner-centered approach. For classroom based learning, we made four identical sessions for classroom based learning and each of those sessions are from 3pm to 4pm. This is the time period while AM shift colleagues finish their work but not yet reach official off time. For simulation training, they are also arranged at 3pm to 4pm. Measurements for the course included pre-post resuscitation knowledge assessment, pre-post self confidence of management of resuscitation and course evaluation.
Result & Outcome :
More than 90% of nursing staff of Department of Nursing of FYKH attended the course. After the course, there were 1. Significant increase in resuscitation knowledge assessment (full mark 6) from 4.6±1.0 to 5.7±0.5 (p< 0.001) 2. Significant increase in self-confidence of management of basic resuscitation from 6.4±1.5 to 8.3±1.1 (p< 0.001) Besides, all participants agree that the course can ensure patient receive prompt and proper resuscitation before doctor’s arrival. In summary, this update course can enhance skill and confidence of FYKH nursing staff in performing resuscitation.
