Introduction :
Transarterial chemoembolization (TACE) is one of the common clinical procedures for patients with hepatocellular carcinoma (HCC) in our setting. A clinical pathway was established to simplify the lists of pre- and post-TACE nursing care. However, it is a challenge for new joined nurses to get used to the clinical pathway especially the complicated medication regime used in TACE. There were numerous medication incidents taken place related to TACE such as giving the medication at wrong time and not withholding oral metformin after TACE. In order to eliminate the medication incident related to TACE, a training program was formulated to facilitate the new joined nurses to get familiar with the TACE clinical pathway since September 2018.
Objectives :
Objectives: All new joined nurses can manage the TACE clinical pathway well with:
1. no medication incident.
2. no missing task in pre-TACE preparation day.
3. handling job sheet of blood taking correctly.
Methodology :
The training program includes:
1. A group briefing with the new joined nurses was held once they come to our unit. An evaluation about their satisfaction level would be collected.
2. A 11-paged TACE handbook was made. The content is about the whole journey of TACE, from pre-TACE preparation checklist to post TACE nursing care as well as the tips and tricks in checking of medication prescription. Precaution about the application of In-patient Medication Order Entry System (IPMOE) was specially highlighted.
3. New joined nurses need to demonstrate the nursing care and medication prescription checking with supervision at least once, an audit using a designated checklist would be done before they take care of patients independently.
Result & Outcome :
There were 15 newly joined nurses engaged in this program, all of them were highly impressed with the training program, they specially highlighted the sharing about the medication incident, explanation by using real cases and IPMOE were very useful. Furthermore, they all had positive feedback on the tailor-made handbook, the tips and tricks about the medication prescription can be served as a good reminder for them practically. On the other hand, all of them passed the audit. No medication incident related to TACE was reported and no report about missing preparation task or blood taking after initiation of the training.
