Introduction :
To ensure the quality of care and reduce the incidence of complications or readmission, it is crucial that primary caregivers receive proper education on nasogastric tube (NGT) feeding before patient discharge home.
Traditional face-to-face demonstration was very time consuming. It was also difficult for caregivers coming for training repeatedly until they could pass the assessment.
Moreover, most of the caregivers are foreign helpers who only speak Indonesian or English. The consequences of miscommunication include (1) Time consuming and difficult to assess the understanding of the caregivers. (2) Prolong training and delay on patient discharge. (3) Leading to severe patient harm if the caregivers practice the procedure wrongly.
A “Smart Approach” education was implemented since November 2019, it consists of (1) standardizing current practice (2) providing comprehensive pamphlets and (3) educating by video with QR codes with Chinese, Indonesian and English subtitle to facilitate caregivers' revision and support at home and (4) bedside evaluation to ensure quality and safety on NGT feeding.
Objectives :
(1) To provide effective educational strategies
(2) To empower the caregiver to deliver the quality and safety care on NGT feeding in community
Methodology :
Prequestionnaires were collected from staffs to analysis the problems of NGT training. The time and sessions used in training were also recorded.
After the implementation, Staffs’ satisfaction survey and individual feedbacks were collected. Additionally, the Caregivers’ satisfaction survey and feedbacks were collected. The time and sessions used in training were compared.
Result & Outcome :
The nursing time used in caregiver training decreased from 34 minutes to 19 minutes per sessions, the total number of sessions required before patient discharge decreased from 8 sessions to 3 sessions averagely.
98% of staffs strongly agreed and 2% agreed the “Smart Approach” improve the efficiency. 97% strongly agreed the “Smart Approach” effectively solve the problem of language barrier.
100% caregivers strongly agreed the QR codes provides a convenience way to revise and support at home. 100% caregivers strongly agreed the “Smart Approach” reduce anxiety and improve confidence in caring of NGT patients.
Conclusion:
The Smart Approach is effective in (1) improve the training quality especially for foreign helpers (2) reduce the training time (3) facilitating discharge (4) empower the caregivers in supporting patients in community.
