Introduction :
Tracheostomy care is a high-risk nursing care. Proper management and adequate communication can prevent adverse consequences or fatal. Maintaining patient airway being a high priority for patients with tracheostomy tube. Thus, frontline nurses should equip with tracheostomy related knowledge and skill and maintain accurate documentation.
Objectives :
The aim of this program is to: -Reinforce safety and quality in tracheostomy care -Facilitate in conducting comprehensive assessment and care planning in tracheostomy care -Save time and efforts for nurses in documentation
Methodology :
1.Provided two identical training workshops in tracheostomy care by Nurse Consultant (Respiratory) to enhance nurses' knowledge and skill including knowledge and care of tracheostomy, complications & risks of tracheostomy, planning care for tracheostomy, emergency management of tracheostomy care. 2.Created a documentation template of Tracheostomy Nursing Note in Clinical Management System (CMS) with the support and endorsement from General Manager (Nursing) and Chief of Service for hospital- wide implementation.
Result & Outcome :
Lectures and workshop in tracheostomy care -Two identical workshops were successfully held in May 2018. A total of 88 nurses in different departments had attended the training with positive feedback. The overall satisfaction score was 4.87 out of 6. 81.5% of participants found the training was useful and met their expectation. Nursing note -Nursing Note (Tracheostomy) template was launched in all wards of Hope of Hope Hospital in August 2018. With a standardized template, nurses could input information in a short time by selecting predesigned options. Moreover, missing information was avoided by just filling in this comprehensive template. -30 patients with Tracheostomy were admitted in 12 wards of Haven of Hope Hospital from July to November 2019. Audit was conducted to assess the effectiveness of the nursing note. The overall compliance rate of the audit was 99.2%. Ward in charge and APN expressed that the nursing note helped supervisor to identify potential risk in a short time. Nurses can systematically and efficiently in reviewing the tracheostomy status of patient with the template Conclusion Through the training workshops, participants had an opportunity to experience the procedures in a virtual environment. Feedback showed that nurses have strengthened their competency in handling tracheostomy. Moreover, tracheostomy nursing note is systematic, clear and easy to follow. Standardized template prevents misunderstanding in the communication loop. It is believed that it can help in minimizing the risk of complications.
