Introduction :
Pressure Injury had substantial impacts on patients’ quality of life. It not only resulted in psychological distress, but also increased patients’ length of hospital stay and exerted huge financial burden in our healthcare sector. According to Hospital Authority (HA) Nursing Quality and Safety Annual Report 2017-2018, the prevalence of pressure injury in HKBH was 0.7 (per 1,000 in-patient bed days occupied). In benching with Group 2 hospitals in HA, the hospital acquired pressure injury rate of HKBH was ranked as second highest.
Nowadays, encouraging patient or their carer participation in care is a growing trend in healthcare as it can highly increase their adherence to treatment plans and ultimately improve their clinical outcomes. For this reason, client engagement talk on pressure injury prevention has been launched in HKBH sicne 2018.
Objectives :
1. To enhance clients’ understanding on causes of pressure
injury.
2. To enhance clients’ knowledge in pressure injury prevention.
3. To provide different types of pressure relieving devices for
clients as reference.
4. To enhance proper application of pressure relieving device
in clients.
Methodology :
The client engagement talk was commenced in August 2018. Teaching material related to causes and preventive methods of pressure injury was prepared. The teaching material was reviewed by the Nurse Consultant of Queen Elizabeth Hospital Orthopedic and Traumatic Department. The material was specifically designed to include big and funny picture for easy understanding and memorize in our elderly clients. The talks were held in 4 medical wards and 15-20 patients and their family members were invited in each session.
In each session, teaching and sharing were done by one of the 2nd tier wound nurses in HKBH. The teaching content included definition, causes, common affected locations and preventive methods of pressure injury. There were also question & answer sessions for the participants to share their own experiences and ask questions.
3D model on skin anatomy and bony prominence areas were used to facilitate clients’ understanding. Besides, different pressure relieving materials were introduced. Client satisfaction survey was conducted afterward to collect clients’ feedback for later quality improvement work.
Result & Outcome :
1. Total of 10 talk were held till January 2020.
2. 191 patients and their family members attended.
3. The average score of the satisfaction survey was 4.83/5.
4. Other suggestions from the participants included:
-adding skeleton model in the talk for illustration
-teaching about the ways to massage skin
-delivering Indonesian leaflet about pressure injury
-educating old age home staff about pressure injury knowledge
