Introduction :
Phlebitis is one of the most common complications of peripheral venous cannulation, result in life threatening complication such as septicaemia. Peripheral venous cannulation care bundle implemented and visual infusion phlebitis (VIP) score were introduced as the standardized assessment tool.
Objectives :
This study aims to maintain a high standard of nursing care to prevent venous cannula related to infections; to investigate and care the incidence of phlebitis and evaluate factors contributing to the development of phlebitis.
Methodology :
Direct observation was adopted. A total of 100 samples were selected, 50 samples were collected as pre-intervention. A briefing session on Peripheral Venous Cannulation Care Practice was given to all ward staff in order to reinforce and refresh the updated practice. Another 50 samples were collected as post intervention. 28 nurses attended the briefing sessions and 27 nurses were being observed before and after the briefing.
Result & Outcome :
From March 2019 until now, the incidence rate of phlebitis is 0%. The overall compliance rate was improved from 60% to 81%. There was significant improvements on the documentation of VIP score; the date of changing insertion site every 96 hours. The results showed partially achieved but required continuous reinforcement that corresponding to the audit result of the infection control team. Nurses should increase the awareness on the early signs of phlebitis, change the insertion site if any doubt and remove it promptly if no longer in use.
The aims of this exercise is to provide guidance to nursing staff in M&G male rehabilitation ward on caring of patient requiring insertion of peripheral intravenous line. Phlebitis is an ongoing problem in our clinical practice. Proper nursing care and daily inspection of catheters with special attention to high risk case is needed for preventing of phlebitis.
