Introduction :
It is truth that nurses always express depressed in heavy workload. Regular routine work makes them feel boring and lack of motion. The role of rehabilitation nurse is promoting physical training and providing education to patients and care givers. Good clinical outcome for patients can better promote patients discharge to community. The nurses’ attitude and motivation in deliver care will influence the quality of care. Reintegrated nursing team is promoted in rehabilitation settings for improve department service and staff engagement.
Objectives :
Reform the service model with nurse leader as supervision and mentoring team members for improve the clinical outcome and increase staff engagement in conductive education team.
Methodology :
Pilot program is conducted in conductive education team from Jan-Dec 2019 with themes of “TEAM”: T: Training, E: Evaluation, A: Appreciation and M: Motion

Nurse’s leader has related professional qualification and team members have attended related courses. According to the theme “TEAM “, nurse’s leader provides refreshing training in the beginning of year and evaluation at the end of year. Regular mentoring and monitoring for measure team performance in related services. Support with recognition offers to team members. Leader and members has a clear fitting plan of whole year function through open discussion.
Result & Outcome :
Staff feedback survey is used for satisfaction of the service model. There are total six nurses and one nurse leader. The survey covered several aspects related to group service design, mentoring styles, supporting resources, benefits of reform service model. The results of survey are all positive and members recommend new service model to other functional team. Thus, nurses agree the new service model benefits to clinical outcomes.

Nurse leader and team members should maintain close communication and same objective in team. Good communication can provide more brainstorming and improvement in related service. Further, members would participate in the service and demonstrate happy atmosphere in clinical workplace.
