Nurses competence and enhancement on the care of patients under Therapeutic Plasma Exchange (TPE) at renal unit

Introduction :
Therapeutic plasma exchange is an extracorporeal blood purification technique designed for the removal of macromolecules including pathogenic auto antibodies, cryoglobulins and endotoxins. Patients suffering from hematological, neurological, or autoimmune diseases could be benefit from TPE treatment. TPE service has been introduced to Caritas Medical Centre renal unit in 3Q 2019. For the preparation of the new TPE service, an onsite enhancement training program was launched to enhance renal nurses’ confidence and competence in taking care of patients under TPE treatment.
Objectives :
(1)To standardize the content of TPE education program; (2) To enhance competency and knowledge of qualified renal nurses to perform TPE treatment; (3) To improve patient safety and high quality of nursing care through onsite training program of qualified renal nurses.
Methodology :
A TPE onsite enhancement training program was designed and launched in early June 2019. 20 qualified renal nurses had participated in the training. The program includes a lecture, a training video and workshops for participants to gain hands-on experience. A ‘pre-test’ and ‘post-test’ was employed.
Result & Outcome :
The overall results of the ‘pre-test’ and ‘post-test’ were 94.1% vs. 100% respectively. Questions of the test covered the aim, indication, nursing care and treatment process of TPE treatment.



Question covering TPE nursing care resulted in ‘pre-test’ and ‘post- test’ 60% vs. 100% respectively; it reflected that the knowledge of TPE nursing care was raised after training. The last question covered the treatment process (pre and post- test: 80% vs. 100%).



Upon completion of the TPE training program, all nurses had hands-on knowledge on caring TPE patients. Participants agreed that TPE training program met their expectations and needs and they expressed that they were more confidence in providing quality treatment and improved job satisfaction as well.



Qualified renal nurses were able to identify risks and provide safety TPE treatment to patients, which can therefore, aligned the best practice in performing TPE nursing care. In the future, half- yearly audit will be conducted to improve and maintain the high quality nursing care on TPE.
