Introduction :
In-situ simulation integrates the real clinical environment and clinical cases scenarios into training, to improve reliability and safety in high-risk areas. Trauma resuscitation demands high-risk and time-pressured, requires an effective multidisciplinary team with strong clinical knowledge, leadership, coordination and problem-solving skills. In order to improve clinical service and at the same time ensure clinical safety and quality service, in-situ simulation training in PWH A&E trauma room was initiated.
Objectives :
1. To enhance the clinical knowledge and skills on management of trauma patients. 2. To facilitate the understanding of trauma team concept including leadership, coordination and communication and decision making skills.
Methodology :
We invited clinical experts and simulation instructors to take up the role as facilitators. Trauma team members whom included A&E physician and nurses, General surgeons, Orthopedic surgeons, Neurosurgeons, Cardiac Thoracic surgeons etc. are invited to participate in the in-situ simulation training. The training was held in A&E trauma room and utilized real life case scenarios as training materials. After simulation training and debriefing, a theoretical input was introduced to reinforce the important learning points. The training covered the high risk area, difficult cases management and new developments or guidelines in trauma. The trauma team concept including leadership, coordination and communication and decision making skills were also emphasized. Some sections also had hands-on skill stations e.g. airway management, open thoracotomy, log roll, application of pelvic binder, traction splint etc. The in-situ simulation training were scheduled every 4-6 months
Result & Outcome :
In situ simulation trainings were run from 14/07/2016 - 10/10/2019 in A&E Trauma Room 2. There were total 183 participants which included 102 doctors and 79 nurses. A total of 114 evaluations were collected and the feedbacks were very encouraging. 100% of participants commented the learning points were clear; 99% had better understand the trauma team concepts; 97% commented that training was well organized and 96% of them claimed that their learning was significant enhanced.
