Introduction :
Reducing medication incidents play an important part of maintaining patients’ safety in our department. Among the medication incidents, drug allergy can cause permanent harm or death to patients. From 1/2018-5/2019, there are 3 cases of medication incidents relating to drug allergy happened in AED. The causes of medication incidents include lack of drug allergy awareness, such as not comply to medication administration guidelines, or lack of drug knowledge. Also, during resuscitation, it may reduce the alertness of staff when strive for urgency. It is also noticed that some patients do not know their drug allergy status. Therefore, a CQI project on safe medication administration is launched in order to minimize drug allergy related incidents in our department.
Objectives :
1.To reduce medication incidents in AED 2.To promulgate a safe culture on drug administration 3.To enhance the medication allergy awareness of nurses 4.To improve patient safety in AED
Methodology :
Introducing a systematic approach of drug administration including a slogan “APSE” cue cards and “Speak Up” culture when counter checking medication. A – Allergy P – Prescription S – 5 RightS E – Expiry Date Training: A training video “派藥方程式” on safe medication administration was prepared and showed to all nurses. Evaluation: Audit on oral and IV medication administration after watching video and a staff satisfaction survey were conducted after launching of project.
Result & Outcome :
1.Results of the audit on oral and IV medication administration were 100% compliance to the standard criteria of Oral and IV medication administration under “APSE” and “Speak Up” Culture. 2.Return rate of staff survey was 91% (40 returned/ 44 issued). Among the 40 returned survey, 100% show that the project is helpful, easy to follow and will continue to adopt in future.
