Introduction :
Pressure injury (PI) made a significant impact on health-related quality of life for clients and increased healthcare costs. Pressure injury incident rate was one of nursing quality indicator (NQI) to monitor the succeeding in preventing PI and areas in which care could be improved. However, it showed increasing trend in our department. Therefore, an enhancement program on pressure injury prevention was proposed and implemented.
Objectives :
(1) Enhance knowledge of nursing staff in pressure injury and its preventive measures (2) Reduce the incident rate of pressure injury in our department
Methodology :
Serial interventions were performed after relevant possible factors identified such as new graduated and recruited nursing staff, misconception of pressure injury and inadequate awareness on medical devices related pressure injury (MDRPI) by orthopaedic related prosthesis, orthosis and tractions. The relevant interventions included orientation program to new comers, electronic educational platform and refreshment session to staff with updated training materials, pressure relieving devices replacement and adoption.
Result & Outcome :
Results: Total 15 and 25 new comers attended the orientation program on "Pressure Injury Prevention and Nursing Management of Clients with Orthopaedic Related Medical Devices and Tractions" in September 2019 and 2020 respectively. Staff refreshment session on "Pressure Injury Prevention and Management" was performed in September 2019. Booklet of updated pressure relieving materials and devices was introduced. Training of PI prevention, including assessments, orthopaedic related medical devices and tractions, observations, skills of pressure relieving materials and devices application and relevant managements, was implemented. Total 18 nursing staff participated and passed the quits. Bedsides, all nursing staff (total 72) had completed the e-learning on "Pressure Injury Prevention for Nurses" with records in June 2019. Although the training session was suspended due to the pandemic in 2020, clinical education and supervision continued. Pressure relieving mattress had been changed in 3 wards from 60% (2019) to 80% (2020). Knowledge of nursing staff in PI and its preventive measures was enhanced, it showed from the quits and satisfaction survey. The pressure injury incident rate was reduced 43% in the NQI report of 2019 and further reduced 50% in the third quarter report of 2020. Conclusions: Through the program, training accompany with resources improvement (pressure relieving materials and devices) enhanced knowledge, skill and awareness of nursing staff in pressure injury prevention, especially in MDRPI from orthopedic prosthesis, orthosis and tractions. It also aroused their attention of basic nursing care in moisture, friction, shear and pressure prevention in addition to good hygiene and nutrition maintenance. Hospital acquired pressure injury incident rate was alleviated in our department.
