A Pioneer for Collaborative Care during COVID-19 Crisis — Experience of CGAT Support in “Site Y” Quarantine Camp

Introduction :
In the third wave of COVID-19 pandemic (started from July 2020), sereval outbreaks occurred in RCHEs and RCHDs. Close and non-close contact residents of these homes were evacuated to Site Y (Cheung Muk Tau Holiday Centre for the Elderly) so as to receive quarantine and medical surveillance care respectively. PWH CGAT took up a pioneer mission for caring those residents in this quarantine camp setting alongside with high level of infection control measures.
Objectives :
(1) To provide extended CGAT service scope to quarantine camp
(2) To keep vigilant in monitoring health and safety of all residents in Site Y during quarantine.
(3) To prevent cross infections of COVID-19 among residents and the healthcare providers in Site Y.
Methodology :
(1) A designate team comprising nurses and doctors was formed to provide support to Site Y with advice input from hospital infection control team.
(2) Liaised with Site-Y manager for Wi-Fi arrangement and remote CMS workstation preparation for telemedicine and clinical support.
(3) Ensured operation of Site Y complying with high level of infection control measures including provision of adequate infection control training to two non-professional private care team, particularly the proper technique of gowning up and down PPEs, demarcation of clean zone and dirty zone, clinical waste and laundry management, workflow in medication administration or meal delivery, inspection of carer quarter and meal arrangement with social distance and board separation, allocation bed and room according to the exposure risks of residents and daily surveillance of care team and resident for any respiratory symptoms.
(2) On-site health assessment was provided to residents on arrival of quarantine site and tele- support and telemedicine for ad-hoc consultation. Liaised with Medical Surveillance Team (From Department of Health) for residents with deterioration for early and coordinated hospital admissions
(3) Daily situation report to acute hospital departments and management for prompt communication and reporting; and liaised with hospital infection control team for admitted resident management.
Result & Outcome :
From 23 July 2020 to 28 October 2020, total 124 residents from 6 RCHEs and 1 RCHDs were evacuated to Site Y in which the homes located in different clusters were confirmed with COVID -19 outbreak.106 residents were close contacts and 18 were non-closed contacts. PWH CGAT doctors supported all residents with tele-medicine consultation, while CGAT nurses provided over 1000 attendance in the quarantine camp. 4 residents (3%) who stayed in Site Y subsequently confirmed with COVID-19. The mean age of these confirmed patients is 92.75 years old, with a mortality rate 1.6%. Our team work approach eventually resulting no significant outbreak in Site Y;no private care team’s worker infected with COVID-19.
