A retrospective review on the effectiveness of 365-stroke rehabilitation program in Tai Po Hospital

Introduction :
The 365-stroke rehabilitation program was first launched in Tai Po Hospital on 1 October 2018. Rehabilitation services were provided to stroke patients on weekends in addition to regular daily basis. Improvement was noted in clinical and service outcomes with adoption of the new service model.
Objectives :
To evaluate the benefit of the 365-stroke rehabilitation service model
Methodology :
Stroke patients admitted to TPH from 1 October 2018 to 30 September 2019 (365 group) receiving daily stroke rehabilitation services was included. Patients’ clinical outcome including Modified Barthel Index (MBI), Functional Test for the Hemiplegic Upper Extremity (FTHUE), Montreal Cognitive Assessment Hong Kong Version (HK-MoCA), and the service outcomes such as the response time to referrals and patients’ length of stay were reviewed. The data were compared to those of the non-365 group which were captured in 1 October 2017 to 30 September 2018 before the launch of the 365-stroke rehabilitation program.
Result & Outcome :
A total of 271 patients (mean age 73.41 years old) was recruited in the 365 group. Mean difference of MBI score on admission and discharge were 9.46 in the 365 group. There was an increase of 23.2% when compared to that of the non-365 group (mean MBI difference = 7.68). The FTHUE mean difference in the 365 group was 0.98 whereas the non-365 group was 0.55. There was an increase of 156% in the mean difference of HK-MoCA score of 365 group on admission and discharge when compared to the non-365 group (365 group mean difference = 1.56; non-365 group mean difference = 0.61). The mean response time to referrals was decreased from 1.59 days in the non-365 group to 0.93 day in the 365 group. The length of stay of patients were slightly higher in both 365 group (19.48 days) when compared to the non-365 group (18.94 days), which might be due to the high occupancy and turnover rate of wards bed during the winter surge period in 2017-18. To conclude, the new rehabilitation service model was effective in achieving better functional recovery, hand function and cognitive function in stroke patients. The service outcome was also enhanced.
