Introduction :
Devastating obstetrics emergencies are uncommon events, but it could occur at any time and anyplace. These situations are unanticipated and always happen rapidly. It is highly important that all midwives should have well equipped with skills and knowledge to manage infrequent, but deadly obstetric emergencies. Furthermore, it is evidence-based to support obstetric emergency drills can provide a safe simulated environment for learning and reduces training variability and increases standardization. Therefore, a series of obstetric emergency drills are designed with scenarios are implemented in our Obstetrics and Gynecology Department, the aim is to enhance midwives' competence and ensure best practice.
Objectives :
1. To identify the causes of particular obstetric emergencies 2. To strengthen the teamwork and enhance midwives’ capability to respond 3. To improve effective communication skills
Methodology :
The drills are implemented in the antenatal ward, delivery Suite, postnatal ward and gynecology ward from April of 2020 to October 2020. The topic selected in each ward is high risk but low volume. The scenarios are self-designed and the checklists are according to the department’s guidelines and protocol to plan. Target participants including midwives, student midwives and supporting staff, they are assigned as actress and observers.
Result & Outcome :
1. Antenatal Ward - Handling imminent delivery in the vicinity of the special block The overall compliance rate is 99.72% Recommendation: to familiar with the logistics to transfer the patient to the delivery suite and to get extra help from her husband. 2. Delivery Suite - Resuscitation for Maternal desaturation The overall compliance rate is 98.3%. Recommendation: to reinforce the use of the Airvo2. 3. Postnatal Ward – Eclampsia The overall compliance rate is 100%. Recommendation: to familiar with the anticonvulsive drug and the use of the syringe pump. 4. Gynecology ward - Rupture ectopic pregnancy The overall compliance rate is 98%. Recommendation: to train staff to set up the blood giving set. Conclusion: Individual’s strengths and weaknesses of performance are identified in the drills. Team situation awareness and leadership roles are enhanced. However, not all the staff could participate in the drill during the working hour. It is suggested that the format could be changed from “hands-on drill” to “oral drill” so that more staff could have chances to participate.
