User based digitalization of SSC with CMS integration to facilitate patient hand over in OT nurse station

Introduction :
Patient safety is the highest priority in operation theater service. There are many sentinel event related to operation theater including the wrong patient or wrong-site surgery, retained instrument of other material, incompatibility blood transfusion and wrong medication etc. In such a pressurized working environment, surgical safety checklist(SSC) is developed to ensure the safety of the patient. However, apart from SSC, there are many other documentations that perioperative nurses need to check and hand over.
In TKOH, perioperative nurses need to ensure each patient arrive OT nurse station with their paper form consent, anaesthestic record and SSC. Afterthat they need to use one CMS computer to check for patient medication order, lab result, radiology film availability. The whole process would involve a lot of copying an electronic document to paper form.
Objectives :
Digitalization of SSC with CMS integration aim to
1. With CMS integration, needed information can be synchronized to the E-form automatically
2. Checkbox checklist can simplify the need of data entry
3. Alert system can be implemented to ensure nurses complete all the required setting
4. Using barcode scanner to reduce human error and minimize typing
5. Promote the use of HA clinical suite (mobile CMS)
6. Using staff card to login to increase user-friendliness
Methodology :
With the help of third party vendor, we designed our digital SSC. The digital SSC is separated into 4 parts including "floor in OT", "sign in theater" , "timeout" and "sign out".
For the "floor in OT" part, with the help of NMI network and HA IPAD, we can access both clinical suite of HA and our local server by using one ipad. Nurses can then using the same ipad connected with a wireless barcode scanner to complete patient documentation checking and fill in the SSC. The rest of the SSC is then integrated with our own perioperative nursing information system. Nurses can then complete the SSC with anaesthetist and surgeon before the start of the operation.
Result & Outcome :
Many of our nurses especially senior nurses gives us the following comment 1. The ipad clinical suite is user friendly and fast responding 2. E-form after Integration with CMS can display needed information automatically, reduce the time to search for the data 3. Alert system can remind them to complete all the checkbox and required patient checking 4. The barcode scanner is so useful for fast access to patient information 5. Paperless of those forms could be environmentally friendly
