Introduction :
ICU was a busy and demanding working environment, patients under ICU care could easily develop stress and anxiety that subsequently cause some undesirable responses, such as hallucination and delirium. Sedatives and analgesics were commonly used to calm down the patients. However, adverse reactions, such as delayed weaning and delirium were reported. Literatures also suggested patients has not relieved stress and anxiety even with the administration of these drugs. Therefore, music therapy was sought for achieving the soothing effect instead of using pharmacological way.
Objectives :
1)To reduce ICU patient’s anxiety and stress level by music therapy; 2) To validate if music therapy is practical and feasible in NDH ICU
Methodology :
Classical music would be played to participants on Day 1 and 3, while Day 2 and 4 were rest day, which means participants were allowed to rest without any music being played. Music would be played twice a day, morning and evening. Subjective and objective data were collected before, during and after the music start.
Result & Outcome :
From September 2019 to November 2019, total 20 patients were invited. 5 of them rejected to join. 4 of 15 completed the project, and the rest of them completed 1 to 3 days after started the project. No specific trend could be concluded for participants’ blood pressure, heart rate, and respiratory rate. 12 out of 15 participants were found to have a drop of RASS from to -1 during music was playing; others keep the same level at 0. A general decreasing trend of visual-analog scale (VAS-A) of participants were noted during music was playing, while VAS-A remained unchanged during their rest day. Conclusion: Music therapy was useful in reducing anxiety of patient in ICU, although it could not be reflected by most physiological parameters. Recommendation: 1) Increase music options based on patients’ preference; 2) Increase music timeslot for participants instead of twice per day; 3) Implement during night time for sleep quality improvement
