Staff Wellness Program: Cardiovascular Risk Assessment For Staff In Tseung Kwan O Hospital, A Ten-Years Review

Introduction :
The prevalence of coronary heart disease (CHD) is increasing worldwide and it was the third leading cause of death in Hong Kong in 2017. Staff is an important asset of the hospital and regular activities in promoting staff wellness have been organized in Tseung Kwan O Hospital (TKOH) over years. We have conducted a cardiovascular risk assessment for all staff in 2019. We also compared the results in 2009 and 2014.
Objectives :
1. To assess staff health status by CHD risk assessment
2. To provide multi-disciplinary group education and medical consultation according to the risk profile
Methodology :
All TKOH staff were invited to join the program via online or in-person registration. Enrolled staff were assessed with measurement of blood pressure, body mass index(BMI) and waist circumference. Fasting blood sugar(FBS) and lipid profile were checked. Ten-years CHD risk was calculated by atherosclerotic cardiovascular disease risk calculator according to American College of Cardiology/American Heart Association guideline. Staff with moderate-CHD risk(M-CHD) were counselled on lifestyle modification by multi-disciplinary talk while high-CHD risk(H-CHD) staff were referred to designated medical clinic. Evaluation questionnaire was conducted at the end of program.
Result & Outcome :
From June to September 2019, 789 staff were enrolled, 664(84%) were female, with median age of 47. Fifty-seven participants(7.2%) had hypertension, 27(3.4%) had diabetes mellitus(DM) and 26(3.3%) were smoker. The baseline characteristics were similar as those in 2009 and 2014. There were 356 participants(45%) with overweight (BMI≥23kg/m2), which was less compared with 2009 (55.5%) and 2014 (46.9%). One-hundred participants(12.7%) got impaired fasting glucose (IFG, FBS>5.7mmol/L) and 9(1.1%) had newly diagnosed DM (FBS>7mmol/L), which was slightly decreased compared with 2014 (13.7% IFG, 1.6% DM) but higher than 2009 (8.6% IFG, 2.2% DM). Six(0.8%) and 18(2.3%) participants had H-CHD and M-CHD risk respectively, the rate was higher compared with 2014 (1.2% M-CHD) but much lower than 2009 (2.2% H-CHD and 11.5% M-CHD). Twenty participants were invited to the multi-disciplinary talk and 7 to the designated medical clinic. Majority (>90%) staff were very satisfied with this program. They agreed the information was very useful and wished to join the relevant program next time. Conclusion The CHD risk was low among TKOH staff while overweight and IFG were major concerns. Continuous promotion on lifestyle modification to staff is paramount for healthy workforce. Regular CHD risk assessment for staff is suggested.
