Introduction :
With the continuous increase of admission rate and the fragility of our patients, there is always a great demand of home visit service. By identifying and eliminating environmental barriers, so as to prepare a modified home environment to facilitate our patient safe discharge home and aging in place.
In 2017, 456 home visits were arranged and the demand was increasing. In order to facilitate the quality service as well as the effectiveness, the work flow of the home visit had been studied and home visit reports had been audited.
Objectives :
(1) To improve the efficiency in the home visit process
(2) To standardize the format of home visit report
(3) To updated the knowledge about home adaptation and modification.
Methodology :
1. Information of flat floor plan for private and public housing were collected, therapist could bring along during home visit and record the home environment and furniture layout onsite.
2. Commercial software was adopted for patient home floor plan drawing and a library of furniture icon was established to standardize our floor plan drawing.
3. Home visit bags with relevant equipment were ready to use and allocated in General Office. Scheduling for checking, cleaning and item refilling of home visit bags were arranged.
4. The template of requisition for home modification to Housing Authority was set and standardized.
5. 6 sessions of in-service training were provided to update knowledge and enhance the standard of providing home visit.
Result & Outcome :
1. The floor plan inclusion in home visit report was raised from 58% in 2017 to 100% in 2019, while percentage of proper ratio and outline of floor plan had improved from 63% in 2017 to 95% in 2019. 2. By equipping the commercial software, the time in preparing a floor plan was greatly decreased from 45 mins to 20mins. 3. The utilization of home visit bag was 100%. 4. The requisition for home modification has been standardized in subsequent audit. 5. After the enhancement, the number of home visit offered was boosted up from 456 nos. in 2017, to 520 nos. in 2018 and finally up to 652 nos. in 2019.
