Moving from a Structured Enhancement Workshop to a Guided Self-Learning Approach for Patient Care Assistant (PCA) in General Out-patient Clinic

Introduction :
PCAs are the backbone of the clinic striving to deliver quantifiable and quality nursing services. There is a trend of rising demand on aligning and reinforcing PCAs’ daily clinical practices. A co-planning workshop as experiential learning in the aide of nurses is designed to help them develop conceptual understanding of the principles of professional knowledge and skills.
A self-engagement and empowerment workshop is conducted which serves as a change agent aiming at helping PCAs gain a better understanding of effective clinical implementation, as well as strategies for sustaining proper practices in the clinics.
Objectives :
The key objective of the workshop is to ensure PCAs’ work quality and standard by clarifying misconceptions in addition to the application of correct and appropriate contexts in clinic routine practices.
Methodology :
The clinic-based activities are arranged to review PCAs’ skill-oriented performance, to scrutinize and evaluate their compliance with the standard healthcare procedure and guidelines.
A set-program framework under the refined learning activities and rubrics is co-planned. The workshop is organized at four interconnected phases. Phase 1 is to obtain a baseline assessment by using a designed pretest questionnaire with 24 multiple-choice questions. Phase 2 is to refresh the workflow at different game booths in focused areas of Self-Blood Pressure Monitoring, blood taking, body temperature measurement, validation of thermometer, infection control, specimen collection, fever cohort room negative pressure testing, Risk Assessment Metabolic Program (RAMP), medication fridge temperature maintaining, Visual Acuity checking and wound management. Phase 3 is to test the knowledge gain after joining the game booths by using the identical designed questionnaire at Phase 1 as a posttest assessment. Phase 4 is a DOM-led review and an overall evaluation of the questionnaires to review the effectiveness of the workshop.
Result & Outcome :
36 participants return their pretest questionnaires, posttest questionnaires and workshop evaluation forms. In Phase 1 (pretest), no participant gets full marks while in Phase 3 (posttest), 1 participant gets full marks. Comparing pretest (Figure 1) and posttest (Figure 2) results, there is significant staff knowledge improvement from 8.33% to 20% in areas of RAMP, infection control and body temperature measurement. There is however no improvement in area of blood pressure monitoring, visual acuity checking and wound management. The mission of the workshop is to support and foster PCAs so as to enhance their knowledge towards competency. There is noticeable change and development in peer evaluation and sharing of views. Positive reinforcement through workshop-wide awards disseminate good practices in Self-directed Experiential Learning. A core learning outcome for enhancing the learning capacity of PCA of the nature, process and skills is reflected.
