Introduction: :
Seasonal Influenza is a persistent risk to the health care service of Hospital Authority (HA) which leads to patient prolonged waiting time in AED and increased inflow of patients to medical wards during winter surge period every year. The epidemiology of influenza transmission is influenced by multiple factors including the transmissibility of virus type and the susceptibility of the population. Government vaccination program (GVP) for health care workers has been implemented in HA Hospitals in the past years and the intervention is proofed could enhance the immune response of health care workers. Thus preventing influenza transmission amongst patients in hospitals and reduce the mortality rate of frail elderly and palliative care patients with low immune resistance.
Objectives: :
To motivate staff in Bradbury Hospice (BBH), Cheshire Home Shatin (SCH) and Shatin Hospital (SH) to Join Government vaccination program(GVP) and enhance their immune response to reduce influenza transmission for protection of frail elderly and palliative care patients in BBH/SCH/SH.
Methodology: :
With the integration of 3 hospitals: BBH; SCH and SH in year 2017 , we have enhanced the hospital governance structure . Local hospital Government Vaccination Program (GVP) workgroup has been established and led by hospital management team. We have invited multiple professionals and different ranks of staff to join as GVP Ambassadors committing GVP promotion in their own team at department level. HCE has set clear targets of vaccination rate 65% for BBH and 60% for SCH and SH. 10 Strategies are formulated into 8 actions and 2 organized hospital activates are implemented including :(1) GVP promotion counter outside staff canteen; (2) Staff express of interest;(3) Departmental contest;(4) Vaccination on wheels;(5) Staff referral incentive program;(6) Staff small group interview;(7) Engage young generation-Y generation in video production; (8) Staff incentive gifts. There are also 2 hospital activities: GVP Ambassador Oath taking ceremony and GVP Kick Off ceremony to motivate staff engagement in GVP.
Result & Outcome: :
BBH , SCH and SH have attained the highest vaccine rate amongst all HA hospitals in 2018/19 The vaccination rate in 18/19 was 81.3% (BBH), 72.2% (SCH) and 72.0% (SH) which has exceeded the original target. The high GVP vaccination rate is sustained in 2019/20. As at December, 2019, the GVP vaccination rate is 96.0% (BBH) , 67.87% (SCH) and 69.79% (SH). The staff average sick leave rate is also reduced from 8.50d ( Apr 2017 to Mar 2018) to 7.30d (Apr 2018 to Mar2019) showing that staff is getting benefit from the GVP . Finally, there is minimized influenza transmission amongst patients and there is no severe influenza outbreak in BBH/SCH/SH in the past 2 years.
